Adaptogens

Reishi Mushroom (Ganoderma lucidum)

Evidence: moderate_human

Mechanism of Action

Reishi contains triterpenoids (ganoderic acids, >130 identified) and polysaccharides (beta-glucans). Ganoderic acids inhibit histamine release, 5-alpha-reductase (DHT reduction), and ACE (angiotensin-converting enzyme). Beta-glucans (1,3/1,6-D-glucan) activate innate immunity via Dectin-1, CR3, and TLR-2 receptors on macrophages, dendritic cells, and NK cells. Reishi modulates the adaptive immune system by promoting Th1/Th2 balance and enhancing regulatory T-cell function. It also supports GABAergic signaling (calming effect), inhibits NF-kB, and modulates gut microbiome composition. Triterpenes support liver function via Phase I/II enzyme modulation.

Dosing Protocol

Standard: 1000-3000mg daily (dual extract: hot water + alcohol for both polysaccharides and triterpenes)

Maintenance: 1000-2000mg/day

Administration: oral

Timing: Evening preferred (calming, supports sleep). With meals.

Duration: ongoing or cycle 12 weeks on, 4 weeks off

Notes

Reishi is the 'calming' mushroom — contrast with cordyceps (energizing) and lion's mane (cognitive). The hepatotoxicity reports (2018-2024) are primarily associated with powdered whole mushroom products, not properly extracted forms. Dual extraction is non-negotiable for full therapeutic benefit. Traditionally called 'the mushroom of immortality' in Chinese medicine. The 5-alpha-reductase inhibition makes it relevant for DHT-related conditions (BPH, hair loss) but may reduce androgenic signaling in men seeking testosterone optimization — consider this when stacking with Tongkat Ali or Fadogia.

Stacking

  • Ashwagandha
  • Magnesium
  • Lion's Mane
  • Turkey Tail

Interactions

  • Anticoagulants/antiplatelets [MEDIUM] — Reishi has antiplatelet activity — additive bleeding risk
  • Immunosuppressants [MEDIUM] — Immunomodulatory — may alter immunosuppressive drug requirements
  • Antihypertensives [LOW] — Additive blood pressure lowering

Contraindications

  • Autoimmune conditions (active flare — immunomodulatory)
  • Bleeding disorders
  • Pre-surgery (discontinue 2 weeks prior)
  • Organ transplant recipients on immunosuppressants

Side Effects

  • GI upset
  • Dry mouth/throat
  • Dizziness
  • Nosebleeds (antiplatelet effect)
  • Rare: hepatotoxicity (powdered whole mushroom products)
  • Skin rash

Source Quality

Dual extraction (hot water for beta-glucans + alcohol for triterpenes) is essential — neither extraction alone captures the full spectrum. Fruiting body, not mycelium on grain. Triterpene content should be >2% and beta-glucan >20%. Cracked spore reishi provides additional triterpene-rich spore oil. Brands: Real Mushrooms, Nootropics Depot (dual extract), Four Sigmatic (beverage form).
