Nattokinase

Mechanism of Action

Nattokinase is a fibrinolytic serine protease extracted from natto (fermented soybeans). It directly degrades fibrin (the structural protein of blood clots) through four mechanisms: direct fibrinolysis, enhancement of endogenous tPA (tissue plasminogen activator) production, conversion of prourokinase to urokinase, and degradation of PAI-1 (plasminogen activator inhibitor-1). It also reduces blood viscosity and may inhibit ACE (angiotensin-converting enzyme), providing mild antihypertensive effects.

Dosing Protocol

Standard: 2000-4000 FU (fibrinolytic units) daily, equivalent to 100-200mg

Maintenance: 2000 FU/day

Administration: oral

Timing: On empty stomach, between meals or before bed. Morning and evening split dosing for 24-hour fibrinolytic coverage.

Duration: ongoing

Notes

Increasingly popular in cardiovascular and 'spike protein' protocols. NSK-SD form with vitamin K removed is critical when the goal is fibrinolysis without coagulation support. The FU measurement is the relevant dosing metric, not milligrams. Extremely important to screen for ALL blood-thinning medications and supplements before recommending. Consider as part of a post-COVID cardiovascular recovery stack.

Interactions

  • Anticoagulants (warfarin, heparin) [HIGH] — Additive anticoagulant effect — risk of serious bleeding. Contraindicated.
  • Antiplatelet drugs (aspirin, clopidogrel) [HIGH] — Significantly increased bleeding risk
  • NSAIDs [MEDIUM] — NSAIDs impair platelet function; additive bleeding risk
  • SIGNIFICANT interaction with anticoagulants (warfarin, DOACs) — additive bleeding risk
  • Caution with antiplatelet agents (aspirin, clopidogrel)

  • Active bleeding or bleeding disorders
  • Scheduled surgery (discontinue 2+ weeks prior)
  • Concurrent anticoagulant/antiplatelet therapy
  • Hemorrhagic stroke history
  • Soy allergy
  • Active bleeding

  • Concurrent anticoagulation therapy without physician supervision

Source Quality

NSK-SD (Japan Bio Science Laboratory) is vitamin K2-removed nattokinase — important to avoid unintended vitamin K effects on coagulation. Activity measured in FU (fibrinolytic units), not mg. Brands: Doctor's Best (NSK-SD), Jarrow Nattokinase. Must be enteric-coated or acid-resistant for survival through stomach.
