NAC (N-Acetyl Cysteine)

Mechanism of Action

NAC is a precursor to L-cysteine, the rate-limiting substrate for glutathione (GSH) synthesis via glutamate-cysteine ligase. It directly replenishes intracellular GSH, the master endogenous antioxidant. NAC also modulates glutamatergic neurotransmission by stimulating the cystine-glutamate antiporter (system Xc-), influencing extrasynaptic glutamate levels. Additionally, it acts as a mucolytic by cleaving disulfide bonds in mucus glycoproteins.

Dosing Protocol

Standard: 600-1200mg daily

Loading: 1800-2400mg/day split doses for acute detox support

Maintenance: 600mg/day

Administration: oral

Timing: On empty stomach for best absorption, 30 min before meals. Split doses if >600mg.

Duration: Cycle 8 weeks on, 2 weeks off (to avoid potential downregulation of endogenous GSH production)

Notes

Hospital standard of care for acetaminophen overdose. Growing psychiatric evidence for OCD, addiction, and trichotillomania. Some practitioners prefer liposomal glutathione directly for clients who tolerate it. Long-term continuous use debate exists — cycling recommended.

Interactions

  • Nitroglycerin [HIGH] — NAC potentiates vasodilatory effects; may cause severe hypotension and headache

  • Chemotherapy agents [MEDIUM] — Theoretical concern of reducing oxidative-stress-mediated cytotoxicity of certain chemo drugs

Contraindications

  • Active gastric ulcers (may increase gastric acid)
  • Asthma (rare bronchospasm with inhaled form)
  • Concurrent chemotherapy (discuss with oncologist)

  • Potential zinc/copper chelation at very high doses
