Mechanism of Action

Clomiphene citrate is a racemic mixture of enclomiphene (trans-isomer, estrogen antagonist) and zuclomiphene (cis-isomer, weak estrogen agonist) that acts as a selective estrogen receptor modulator (SERM). It competitively occupies hypothalamic estrogen receptors, blocking the negative feedback of estradiol on GnRH pulse frequency. This disinhibition increases pulsatile GnRH release, stimulating anterior pituitary gonadotrope secretion of both LH and FSH, which in turn drives testicular testosterone synthesis and spermatogenesis. The zuclomiphene isomer has a much longer half-life (~30 days vs. ~10 days for enclomiphene), leading to tissue accumulation with chronic use.

Dosing Protocol

Standard: Research indicates 25-50 mg daily or every other day for PCT/HPTA restart protocols. Clinical hypogonadism treatment: 25-50 mg daily.

Loading: Research indicates 50 mg daily for the first 1-2 weeks of PCT, then taper to 25 mg daily.

Maintenance: Research indicates 25 mg daily or every other day for ongoing SERM-based testosterone restoration.

Administration: oral

Timing: Take at the same time daily. Evening dosing may reduce perception of visual side effects. No food timing requirements.

Duration: PCT protocols: 4-8 weeks. Long-term SERM monotherapy: 3-12 months with periodic reassessment. Zuclomiphene accumulation is a concern beyond 6 months.

Notes

Clomiphene is the most commonly used PCT compound and off-label treatment for secondary male hypogonadism. The major limitation is the zuclomiphene isomer's 30-day half-life, which accumulates and can cause estrogenic side effects (gynecomastia, mood issues, visual disturbances) with prolonged use. Enclomiphene is the preferred alternative for long-term use. Required bloodwork: Total testosterone, free testosterone, LH, FSH, estradiol, SHBG at baseline, 4 weeks, and end of treatment. Liver function panel every 8 weeks. Visual symptoms require immediate discontinuation and ophthalmologic evaluation. Medical supervision required.

Stacking

  • hCG (pre-PCT priming before starting clomiphene)
  • Tamoxifen (alternative or complementary SERM in PCT)
  • Zinc (supports testosterone synthesis)

Interactions

  • Aromatase inhibitors [MEDIUM] — Concurrent AI use reduces the estradiol signal that clomiphene blocks; may paradoxically reduce effectiveness of GnRH stimulation.
  • Tamoxifen [LOW] — Often co-prescribed in PCT. Tamoxifen may compete for estrogen receptor binding; some protocols alternate rather than stack.
  • Exogenous testosterone [HIGH] — Exogenous testosterone suppresses the HPG axis, directly counteracting clomiphene's mechanism. Do not co-administer.

Contraindications

  • Active liver disease or hepatic dysfunction
  • Undiagnosed abnormal uterine bleeding (if prescribed to females)
  • Ovarian cysts (females)
  • Known hypersensitivity to clomiphene
  • Pituitary tumor
  • Primary hypogonadism (testicular failure — clomiphene only works in secondary hypogonadism)

Side Effects

  • Visual disturbances (blurred vision, floaters, light sensitivity — due to zuclomiphene accumulation)
  • Mood swings and emotional lability
  • Headache
  • Nausea
  • Hot flashes
  • Gynecomastia (paradoxical, from zuclomiphene's estrogenic activity)
  • Elevated estradiol (zuclomiphene accumulation)
