Mechanism of Action

Biotin serves as a covalently bound coenzyme for five mammalian carboxylase enzymes: acetyl-CoA carboxylase (fatty acid synthesis), pyruvate carboxylase (gluconeogenesis), propionyl-CoA carboxylase (odd-chain fatty acid and amino acid metabolism), 3-methylcrotonyl-CoA carboxylase (leucine catabolism), and methylcrotonyl-CoA carboxylase. It also plays a role in histone biotinylation (epigenetic regulation), gene expression regulation, and cell signaling. Essential for keratin infrastructure in hair, skin, and nails.

Dosing Protocol

Standard: 2500-5000 mcg daily

Loading: 10,000 mcg/day for hair/nail restoration (12 weeks)

Maintenance: 2500 mcg/day

Administration: oral

Timing: With meals. Any time of day.

Duration: ongoing for maintenance; 3-6 months for hair/nail cosmetic benefit

Notes

THE CRITICAL SAFETY ISSUE: High-dose biotin causes false lab results on streptavidin-biotin immunoassays. This can produce falsely low troponin (missed heart attack), falsely abnormal thyroid tests (TSH, T4), and false BNP readings. The FDA issued a safety warning in 2017. ALL clients on biotin >5000 mcg must be instructed to discontinue 48-72 hours before ANY blood work and inform their physicians. This is the single most important counseling point for biotin.

Interactions

  • Raw egg whites (avidin) [MEDIUM] — Avidin in raw egg whites irreversibly binds biotin, causing depletion
  • Anticonvulsants (carbamazepine, phenytoin, phenobarbital) [MEDIUM] — These drugs deplete biotin — supplementation recommended
  • Alpha Lipoic Acid [LOW] — ALA competes with biotin for SMVT transporter — separate dosing

Contraindications

  • CRITICAL: High-dose biotin (>5000 mcg) interferes with troponin and other immunoassay lab tests — discontinue 48-72 hours before any blood work

  • Lab test interference (troponin, thyroid, BNP — potentially life-threatening misdiagnosis)
  • Acne/skin breakouts
  • Mild GI upset
  • Insomnia (rare)
