Ashwagandha (KSM-66)

Mechanism of Action

Ashwagandha's primary bioactives are withanolides (particularly withaferin A and withanolide D). KSM-66 is a full-spectrum root extract standardized to >5% withanolides. It modulates the HPA axis by reducing cortisol output (20-30% reduction in trials), likely through GABAergic activity (withanolides are GABA-mimetic at GABA-A receptors) and by normalizing cortisol receptor (GR) sensitivity. It also inhibits the NMDA-induced neurotoxicity pathway, enhances DHEA-S production, promotes thyroid function (increases T4 to T3 conversion), upregulates antioxidant enzymes (SOD, catalase, glutathione peroxidase), and has demonstrated sirtuin-activating properties.

Dosing Protocol

Standard: 600mg KSM-66 daily (300mg 2x/day)

Maintenance: 300-600mg/day

Administration: oral

Timing: Morning and evening with meals. Evening dose supports sleep quality via cortisol reduction.

Duration: Cycle 8-12 weeks on, 2-4 weeks off (to prevent adaptogenic tolerance)

Notes

The single most popular adaptogen in current health optimization. The thyroid interaction is the most critical clinical concern — ALWAYS check thyroid status and medications before recommending. Liver injury reports (2022-2024) have raised concerns, though most cases involved combination products. Monitor liver enzymes if extended use. The cortisol-lowering effect is one of the best-documented adaptogenic mechanisms in human trials. For testosterone optimization, 600mg KSM-66 showed significant improvements in DHEA-S and testosterone in stressed men.

Interactions

  • Immunosuppressants [MEDIUM] — Ashwagandha is immunostimulatory — may counteract immunosuppressive therapy

KSM-66 (Ixoreal Biomed) is the most clinically studied extract — full-spectrum root, standardized to >5% withanolides via milk-based extraction. Sensoril (Natreon) is an alternative root+leaf extract standardized to >10% withanolides with slightly different withanolide profile (more withaferin A).
