| Standard Dose | 40 mg once daily (for educational context — investigational compound, not approved for any indication) | 200-400 mg/day of standardized extract (4-7% ginsenosides, typically G115 brand) or 1-2 g/day of dried root |
| Timing | Once daily, time of day not definitively established from clinical data. With or without food. | Morning on an empty stomach or with food. Avoid evening dosing due to mild stimulatory effects. Acute cognitive effects noted within 1-4 hours of dosing. |
| Cycle Duration | Phase 2 trial used 12-week treatment duration. Long-term safety data unavailable. | Traditionally cycled 2-3 weeks on, 1-2 weeks off. Long-term continuous use exceeding 3 months is debated. |

Benzylpiperizine-aminopyridine compound that stimulates neurogenesis of human hippocampus-derived neural stem cells in vitro and increases hippocampal volume in vivo. Mechanism is independent of serotonin or norepinephrine reuptake inhibition — fundamentally distinct from traditional antidepressants. Activates the TrkB receptor (BDNF receptor) and downstream Akt/PI3K signaling pathways to promote synaptic plasticity, long-term potentiation, and neuronal survival. Enhances BDNF expression in hippocampal subregions critical for memory consolidation and mood regulation. Originally developed as ALTO-100 (Alto Neuroscience) for treatment-resistant depression with cognitive impairment.

Ginsenosides (primarily Rg1, Rb1, Re) are amphipathic steroidal saponins that modulate multiple neurotransmitter systems: enhancement of cholinergic transmission via increased acetylcholine synthesis and release; activation of PI3K/Akt and CREB/BDNF signaling pathways to promote neurogenesis and synaptic plasticity; attenuation of neuroinflammation through Keap1/Nrf2 signaling and suppression of the NF-kB/NLRP3 inflammasome axis; and regulation of the HPA axis to buffer cortisol response to stress. Protopanaxadiol and protopanaxatriol ginsenosides have opposing modulatory effects on GABA-A receptors.
